Behavioral Health Coach

The treatment Annum provides is personalized to each participant, who will have a support team composed of a behavioral health coach, a therapist and a physician -- all of whom share a passion for helping people improve their own lives. The coach plays an integral role in a participant’s care by partnering with the participant on his or her journey. The coach enhances motivation and compliance with the participant’s treatment plan and acts as the coordinator of care throughout the treatment. Using Annum’s technology platform to proactively and efficiently communicate with multiple participants across various channels (text, chat and phone), the coach offers critically important support directly to people “in the moment” as they work to reach their therapeutic goals. A positive, empathetic and nonjudgmental approach are natural-born traits of the coach, who takes personal responsibility for being each Annum participant’s go-to ally on their journey toward a happier and more fulfilling life.    

 

 
 
 

Responsibilities

  • Conducting phone intakes with prospective participants to assess appropriateness for care
  • Provide motivational support, education and consultation to participants via in-app text
  • Act as the coordinator of care for a small multi-disciplinary team, interacting with both therapists and physicians to coordinate and improve quality of care
  • Reinforce the participants’ engagement with evidence-based (cognitive behavioral therapy or “CBT”) treatment by providing support with CBT-based homework and surveys and encouraging compliance
  • Identify crisis situations and redirect to appropriate avenues
  • Improving population health outcomes by ensuring all participants have access to behavioral health expertise

 
 

Qualifications

  • Masters’ degree or equivalent, but not licensed in the field of counseling/therapy, psychology or social work
  • Experience in an integrated medical setting
  • Familiarity with motivational interviewing, cognitive behavioral therapy and/or acceptance and commitment therapy
  • Ability to demonstrate empathy while remaining steadfast with protocols
  • Possesses an exceptional capacity to multitask, while remaining detail-oriented and organized
  • Comfortable working with an interdisciplinary team
  • Strong communication and teaching skills, including a demonstrated ability to communicate effectively with people regarding sensitive subject matter
  • Ability to exercise appropriate judgement
  • Reliable, trustworthy, flexible, and ability to be discreet
  • ·Self-managed; skilled at managing a high volume of work and prioritizing
  • Spanish speaking skills a plus
  • May need to be occasionally accessible on weekends and evenings
